Learning how to control diabetes is critical to enjoying long-term good health and quality of life. Our diabetes self-management education program brings a team approach to helping those with diabetes find support, education and encouragement to live successfully with diabetes.
Morrow County Hospital offers diabetes support groups and diabetes self-management education classes to help you achieve your best level of health and maintain an active lifestyle.
Diabetes Self-Management Education
Diabetes Self-Management Education (DSME) is an outpatient program for individuals with diabetes or prediabetes. DSME provides educational sessions to help people with diabetes make lifestyle changes to successfully manage their diabetes. We offer a variety of programs to meet the unique and personal needs of each individual.
Morrow County Hospital’s Diabetes Education Program is an American Diabetes Association Recognized Program.
To enroll:
- Have your provider complete a referral form.
- Fax the referral form to our office at (419) 949.3114.
- Our office will contact you to make an initial assessment appointment.

Support Groups
A support group is important and helpful to people with diabetes. A support group provides a venue to meet others who share similar concerns and experiences. Each group meeting will focus on another aspect of the disease, often times featuring guest speakers. This is a FREE Morrow County Hospital outreach program.
